Eligible Health Care Provider Requirements
1. Be considered rural by the US Census Bureau, more info here.
2. Be a rural nonprofit or public health care provider of the following type:
- post-secondary educational institiution offering health care instructions, teaching hospitals, and medical schools
- community health center or health center providing health care to migrants
- local health department or agency
- community mental health center
- not-for-profit hospital
- rural health clinic
- consortia of HCP's consisting of one or more of the above entities
- dedicated emmergency department of a rural for-profit hospital
- part-time eligible entity located in a facility that is ineligible
