To file for your video site's Universal Service rebate, NARBHA telemed staff need a copy of a single month's T1 bill each fiscal year (July – June). If your site has had more than one circuit provider in the same fiscal year, we will need copies of the bill from each service provider. You can mail or fax a copy of your T1 bill to Caroline Valencik, NARBHA, 1300 S. Yale St., Flagstaff, AZ 86001, fax 928-774-5665. Also, please notify Caroline immediately at 928-214-2201 if you see unexpected changes to your T1 monthly bills.
