To file for your video site's Universal Service rebate, NARBHA telemed staff need a copy of a single month's circuit bill each fiscal year (July – June). If your site has had more than one circuit provider in the same fiscal year, we will need copies of the bill from each service provider.

You can mail or fax a copy of your T1 bill to:

Also, please notify Caroline immediately at 928-214-2201 if you see unexpected changes to your monthly circuit bills.
